Care for Elders is a partnership of 85 organizations working under the auspices of The Robert Wood Johnson Foundation’s Community Partnerships for Older Adults Program to create systems change that will improve long-term care and supportive systems provided to vulnerable older adults. The Care For Elders Initiative, under the auspices of Sheltering Arms, has developed a comprehensive, four-year strategic plan to create a system of long term care and supportive services for Houston’s vulnerable older adults. The partnership of 85 organizations is working together to create “systems change” in the delivery of services to Harris County’s 250,000 individuals over the age of 65. The Frees Foundation has provided funding support for the implementation of priority strategies included in the plan. View the Long-Term Strategic Plan (pdf). |
